The Linkages Program provides case management that links individuals with services to help maintain and promote independent living. The goal of the program is to prevent premature or inappropriate institutionalization of at risk individuals.
Monday – Friday: 8:00 am – 5:00 pm
Eligibility Requirements
Available for frail older adults, adults with disabilities, and residents of residential care facilities who are scheduled to be discharged within 30 to 60 days from the date of the referral.
